0plus Gold CardThe Mehlville School District 60plus Gold Card program hosted four events during the 2009-10 school year. At these evenmembers were invited into a school to be entertained by talented students, participate in fun, educational activities, tour the

building and meet District staff and volunteers. Watch for information to come about the 2010-2011 60plus events!

Mehlville residents 60 years of age or older can obtain a 60plus Gold Card by calling the Communications Departm

Waiver, Release of Liability and Hold Harmless Agreement ADULT PARTICIPANTS

(for participants 18 and over)return for being accepted into this program offered by the Community Education Department of the Mehlville R-9 School District (theogram), I:

1. Acknowledge that I understand the nature of the Program, and that I am qualified and in proper physical condition to participate in Program. I further agree that if at any time I believe conditions to be unsafe with respect to my physical condition, the equipment, ofacilities, I will immediately discontinue participation in the Program.

2. Further acknowledge that the program involves the risk of serious bodily injury (including the possibility of permanent disability,paralysis, or death), which may be caused by (a) my own actions or inactions, (b) the actions or inactions of others participating in program, (c) the condition of the equipment and/or facilities at which the Program is located, or (d) the actions or inactions of the enand persons identified below; and I fully accept and assume all such risk and all responsibility for losses, costs, and damages I may as a result of my participation in the Program.

3. Accordingly, I hereby release the Mehlville R-P School District, together with its directors, officers, employees, volunteers, and agefrom all liability, claims, demands, losses, or damages arising out of my participation in the Program; and I further agree that if, desthis release and waiver of liability agreement I, or anyone on my behalf, makes a claim released in this agreement, I will indemnify hold harmless each entity and person released herein from any and all litigation expenses, attorney fees, loss, liability, damage, or cthey may incur as the result of such claim. .

Acknowledge that information provided to Mehlville Community Education about myself may be used by a Mehlville representativany individual or organization identified by Mehlville as needed in order to effectively execute this program.6.
